Ballard Nursing Home, Chicago

The Chicago nursing home received a Type A Violation and notice of a $30,000 fine on July 31, 2009. The nursing home did not properly place a resident’s tracheostomy tube and did not reevaluate the position of the tube or notify the resident’s physician.
