Clinton Manor Living Center-DD – New Baden, IL – 4/10/2006
The Clinton Manor Living Center for the developmentally disabled was fined $5,000 for failure to implement their policies, failure to properly alarm or supervise exit doors, and failure to provide adequate supervision to a resident with a diagnosis of profound mental retardation and a history of self-injurious behaviors. The resident was able to walk out of the facility without staff knowledge.
The Illinois Department of Health produces quarterly reports on nursing home violators.
