Failed to provide adequate supervision and assure the safety of a resident known for trying to leave the facility. The resident, who wheeled herself out of the facility and was later found flipped out of her wheelchair, sustained back and finger fractures.
The Illinois Department of Health produces quarterly reports on nursing home violators.
