A 72-year old Dover man died in 2003 resulting from a nurse at Green Valley Pavilion nursing home inserting a feeding tube into his abdominal cavity instead of his stomach. The man subsequently died sepsis and peritonitis that resulted when food that was deposited in his abdomen poisoned the man.
The nurse was charged with patient abuse and a civil suit is pending. The suit alleges the nurse was aware of protocol that provided nurses were not supposed to replace recently implanted tubes.

Alden Village Health Facility for Children and Young Adults in Bloomingdale was fined $10,000 by the state and given a six-month conditional license after a 23-year old patient was raped and impregnated by a nurses aid.
This is not the first violation for the often troubled chain that operates 31 other facilities in Illinois. Since 1996, this particular facility has been cited by the U.S. Department of Health and Human Services 154 times and at least once every year.
Previous fines, citations and lawsuits at the facility include:
•November 2003 - suit alleging sexual abuse of an 11-year-old mentally retarded girl.
•2004 - $50,000 fine when a 12-year-old boy died after he was trapped between his mattress and crib.
•November 2005 – 19 deficiencies cited by the Department of Public Health

Negligent nursing care is the basis for a lawsuit against Crystal Pines Rehabilitation and Health Care Centers in Crystal Lake, Illinois. Staffing failures are to blame in the lawsuit initiated by a women's family against the home. Allegations include failure of the staff to examine the woman for long periods of time when it was known or should have been known that such a failure would prevent staff awareness of conditions requiring treatment.
This is not the first time Crystal Pines has been charged with nursing home negligence. A similar lawsuit was filed in July 2004 against Crystal Pines by the same Chicago-based law firm. That lawsuit, which is pending, says that the same sort of negligent care caused an 86-year-old woman to acquire bed sores, which led to her death.
