Fined for failure to prevent three female residents from being sexually abused by a male resident. Staff did not know how to respond to and relay three witnessed incidents of sexual abuse to administration and did not follow the male resident’s care plan and recommendations of his psychologist. These failures contributed to a fourth incident of sexual abuse.
