Fined for failure to have in place protective measures before allowing residents on a bus with a driver who slapped a resident. In addition, the facility failed to investigate an incident in which a resident left the facility and to take corrective action to prevent other attempts to leave the facility. As a result, the resident was able to leave the facility again without staff knowledge. The facility has not yet requested a hearing, but is still within the allotted time to do so.
