Danville Care Center - Danville, IL - 5/24/05
Fined for failure to monitor the whereabouts of a resident known to wander and to ensure two exit doors were supervised when the door alarms were deactivated. A resident left the facility without staff knowledge and was found by police near a viaduct and railroad tracks. Following the incident, the resident was fitted with an electronic monitoring device and 15-minute checks were initiated.
