Fined for failure to prevent a resident from leaving the facility unnoticed. Staff were unaware of the resident’s disappearance until notified by a local hospital that she was wandering around the emergency department.

Fined for failure to develop a discharge plan and provide services to supervise a resident when she moved out of the facility. With regard to another resident, the facility failed to implement their policy on reporting abuse, thereby allowing alleged abuse to continue. The incident report finds that staff verbally abused a resident.

Fined for failure to provide adequate nursing care. The facility did not check a diabetic resident’s blood sugar level before giving him insulin. The resident had a hypoglycemic reaction and went into cardiac arrest. Twelve other diabetic residents had problems with the management of their diabetes related to missing blood sugar monitoring, wrong doses of insulin and insulin not given as ordered or not given at all.

Fined for failure to monitor the whereabouts of a resident known to wander and to ensure two exit doors were supervised when the door alarms were deactivated. A resident left the facility without staff knowledge and was found by police near a viaduct and railroad tracks. Following the incident, the resident was fitted with an electronic monitoring device and 15-minute checks were initiated.

Fined for failure to provide adequate nursing care to meet the needs of residents. A nurse did not provide proper diabetic care for a resident whose blood sugar was low. Instead, she gave the diabetic resident sugar-free orange juice, which would not improve his condition, and his physician was not notified. Another resident received insulin twice within a week even though she is not a diabetic and had to be hospitalized both times for hypoglycemic reactions. In addition, a Department surveyor reviewed the records of seven other diabetic residents and found problems with their care.

Fined for failure to provide adequate supervision to prevent a resident from leaving the facility unnoticed. The resident, who had a history of wandering, was found walking down the road about three and a half blocks from the facility. The resident was wearing an electronic monitoring device, but staff did not hear an alarm sound. The resident’s device did not sound an alarm at the exit door staff believes he left through.

Fined for failure to prevent a resident from leaving the facility unsupervised. A nurse arriving for work saw the resident exiting the facility and brought her back inside.
