Nursing home negligence leads to citation at Lebanon Terrace
Illinois Nursing Home Neglect: Lebanon Terrace was fined for failure to prevent a resident from leaving the facility unsupervised. The resident was discovered sitting in a van at a local business, approximately one-tenth of a mile from the facility.
The facility also failed to implement its standard procedures for searching for missing individuals; to have evidence that the resident’s disappearance was thoroughly investigated; to report the results of the investigation to the Department within the required timeframe; to take corrective action; and to have sufficient direct care staff on the night the resident left.
