A resident stabbed another resident at Glenshire Nursing Centre. The Illinois nursing home was fined for failure to adequately supervise a resident, which resulted in him stabbing another resident after an argument. The facility also did not ensure that the safety of other residents in the facility was protected by allowing the resident to return to the facility after the assault for 17 days without a care plan and interventions in place to address his physical aggression and alcohol abuse.

Palos Hills Extended Care, an Illinois nursing home, was fined for failure to prevent a resident, who had a history of wandering, from leaving the facility unnoticed. The resident was returned unharmed after a nearby homeowner informed the facility the woman had knocked on the door. It was later discovered that a door alarm at the facility had been unplugged by a nurse aid because it was too noisy.

A Chicago nursing home, South Shore Nursing and Rehabilitation Center was fined for failure provide adequate supervision to two residents. A resident, who had a history of wandering, left the facility unnoticed. She was found by paramedics and taken to a local hospital. Another resident, who was blind and not able to get around by himself, was dropped off at an appointment without an escort. The resident never saw the doctor because he remained in the lobby until the driver returned.

Lakeview Living Center, a Chicago nursing home was fined for staff’s failure to immediately report an incident of sexual abuse to the administrator. By waiting two days before reporting the incident and placing the resident on one-to-one monitoring, residents of the facility were left at risk for being sexually abused.

Illinois Nursing Home Neglect: Lebanon Terrace was fined for failure to prevent a resident from leaving the facility unsupervised. The resident was discovered sitting in a van at a local business, approximately one-tenth of a mile from the facility.

The facility also failed to implement its standard procedures for searching for missing individuals; to have evidence that the resident’s disappearance was thoroughly investigated; to report the results of the investigation to the Department within the required timeframe; to take corrective action; and to have sufficient direct care staff on the night the resident left.

This Illinois nursing home was fined for failure to provide adequate supervision to prevent residents from leaving the facility unsupervised. These failures resulted in three residents leaving the facility undetected.

Insurance premiums in the nursing home market have risen during the last decade due to negligent care. As a result, nursing homes have sought alternative coverage. Recently, there has been a return of insurance companies to the nursing home market in response to nursing homes tightening up regulations including requirements of more favorable nurse-patient staffing ratios and installation of sprinkler systems. Many carriers today will not take on risk unless long term care facilities meet staffing ratio targets.

Good Samaratin Nursing Home was fined for neglect when they failed to prevent a resident from leaving the facility unsupervised. The facility did not realize the resident was missing until staff received a phone call notifying them that the woman was at a business about half mile away. The temperature at the time was in the teens and the resident was not wearing a coat. The facility also failed to have a system in place to monitor and supervise who enters and exits through outside doors. During the Department’s investigation, the resident was seen attempting to leave the facility twice through a laundry room exit. There has not been an Illinois nursing home negligence lawsuit filed yet.

The resident, who was admitted to the facility the day before the incident, was assessed as being at risk for leaving the facility unsupervised and was fitted with an electronic monitoring. Staff did not recall hearing an alarm sound at the time the resident exited the facility and the device did not trigger the alarm when the resident was returned. He was fitted with a new device.

Winecrest Nursing Center, a Chicago nursing home, was fined for failure to provide adequate supervision and to take all necessary precautions to ensure a resident’s environment remains as free of accident hazards as possible. A resident, who had a history of setting fires when he was upset with his wife, started a fire in his room. The facility took no safety precautions to safeguard and closely monitor the resident after he came back early from being out on a pass because of a personal problem with his wife. The facility also failed to check the resident for smoking materials upon his return.
