Two nurses aides at Chicago nursing home International Village witnessed nursing home abuse when an employee continuously slamming a resident into a wall and cursing at the resident. The incident was not reported immediately and the employee continued to have direct contact with the resident for at least two day after the incident.

The Illinois Department of Public Health (IDPH) publishes a yearly listing of nursing homes in Illinois who have had no certification deficiencies since the last annual survey.

Failure to prevent an Illinois nursing home resident from leaving the facility unnoticed. The resident left the facility unnoticed after stating she was going to bed. She later appeared at a trailer park wearing nothing but underwear and a short-sleeve shirt in cold weather. Lebanon failed to keep the facility’s door alarms on, to contact police about a missing resident and to provide evidence that the resident’s elopement was thoroughly investigated.

In Peoria Illinois, nursing home negligence occurred when another resident identified as at risk for suicide was able to leave The Sharon Health Care Willows facility unsupervised and jump head first in front of a car and die.

At Redwood Manor, an Illinois nursing home, A resident identified as at risk for suicide, who had a history of suicide attempts was able to enter a janitor’s room which was not locked properly and drink a container of cleaning disinfectant. He died shortly later.

The U.S. Fire Administration (USFA) has developed a "command and control" online simulation course to help train emergency responders in handling a fire in a nursing home. The simulation presents the user with a kitchen fire in a two-story nursing home. It is designed to help command officials better understand the problems that may occur within nursing facilities during emergencies. Course objectives include safe removal of all occupants and containment and control of fire in the building of origin.

A rule mandating a criminal history record check for certain non-licensed nursing home staff who provide direct care or supervision to residents will go into effect April 1, 2005 in New York State. Nursing homes will be required to obtain a record check from the U.S. Attorney General prior to employment for any prospective employee, and the employee must sign a sworn statement disclosing any findings of resident abuse or convictions of a crime or violation other than a traffic infraction. Financial burden for this verification is placed on the nursing home as a cost of doing business.

The Governor of South Carolina proposed a 5 ombudsmen increase in the 2005-2006 budget, adding to the current 15 long-term care ombudsmen. The state's senior citizen population is predicted to double in the next 15 years, and triple in the next 25 years and currently doesn't have the 1 ombudsmen per 2,000 beds recommended by the Institute of Medicine. Adding ombudsmen will increase the number of those advocating for resident rights, could result in less nursing home negligence, and may decrease the number of nursing home abuse and neglect lawsuits filed due to negligence.
