Illinois Nursing Home residents at a Colfax facility were victims of inappropriate sexual contact by an employee of the facility. At least one incident was witnessed by a nurse who reported the incident to the DON. The DON, who was married to the perpetrator, failed to thoroughly investigate the incidents and the perpetrator was allowed to continue to work at the facility.

A resident with a history of wandering and whose whereabouts were supposed to be monitored every two hours escaped from the facility and was found frozen to death in an alley behind the Illinois nursing home. The nurses’ notes did not reveal that the resident was being monitored and an IDPH survey revealed that the front door of the facility did not have an alarm.
